By booking this appointment I state that I understand Reiki is a meditative energetic technique that helps with stress reduction and relaxation. I acknowledge that sessions administered are only for the purpose of helping me relax and to relieve stress. Reiki Practitioners do not diagnose conditions, nor do they prescribe substances or perform medical treatment, nor interfere with the treatment of a licensed medical professional. It is recommended that I see a licensed physician, or licensed health care professional for any physical or psychological ailment or condition I may have.

I also understand Reiki Practitioner may place their hands directly on me during the session where Reiki is administered in person.

Exemption of Liability: It is agreed between the client and the therapist, that the therapist shall not be held liable for any personal injury of any nature whatsoever that arises from, or is the result of, or is caused or contributed to by the therapy, or by any failure to continue with the therapy.

When I make a booking to receive a session with Japan Reiki Studio, I acknowledge, understand and consent to the statements above.
